1st MTP Cheilectomy and Fusion Surgery in Montreal

Minimally Invasive Solutions For Foot Pain

Painful, stiff big toes can drastically limit your daily activities and overall quality of life. At the Canadian Private Surgery Clinic in Montreal, we specialize in advanced surgical treatments, including minimally invasive surgery (MIS) for 1st metatarsophalangeal (MTP) osteoarthritis and hallux rigidus, with MIS cheilectomy or fusion procedures, helping you quickly regain comfort, mobility, and independence.

Understanding 1st MTP Cheilectomy and Fusion

The first metatarsophalangeal (MTP) joint, connecting your big toe to your foot, is crucial for walking, balance, and overall foot function.

Damage from arthritis, trauma, or prolonged wear can cause significant discomfort, stiffness and mobility issues. Two primary surgical solutions exist:

  • Cheilectomy: Removal of bone spurs from around the joint, improving motion and alleviating pain.
  • Fusion (Arthrodesis): Surgically joining the bones of the big toe at the MTP joint to eliminate the arthritic joint, offering permanent pain relief when severe joint damage occurs, at the expense of motion of that joint.

Conditions Treated

Our surgeons commonly address the following conditions with these procedures:

Surgical Options: Cheilectomy vs. Fusion

Minimally Invasive Cheilectomy (MIS)

This innovative, minimally invasive technique involves a small poke hole, resulting in less trauma to surrounding tissue, minimal scarring, reduced postoperative discomfort, and faster recovery compared to traditional open methods. Ideal candidates typically have early to moderate joint damage with sufficient healthy cartilage remaining.

MTP Fusion (Arthrodesis)

When advanced arthritis or extensive joint damage is present, a MTP fusion is a great choice for pain control and regaining pain-free active lifestyle. This procedure permanently fuses the bones, providing stability, eliminating the degenerative changes, completely alleviating joint pain, and significantly improving walking mechanics. Fusion is typically performed via traditional open surgery but can also be performed with minimally invasive techniques to enhance recovery.

What to Expect: Before, During, and After Surgery

Before Surgery

Your journey starts with a detailed consultation to thoroughly assess your condition, medical history, and surgical suitability. Our specialists discuss your specific symptoms, examine imaging studies (X-ray, MRI, CT scans), and explain your treatment options clearly.

During Surgery

Cheilectomy: Under local anesthesia and sedation, small poke hole incision is made, and the surgeon precisely removes problematic bone spurs and bony impingement.

Fusion: Under regional anesthesia, an open or a MIS approach can be utilized to remove the damaged, to get both bones across the MTP joint to compress with the aid of a plate and screws or screws only, securely joining the bones to promote stable fusion/healing.

After Surgery

Recovery protocols include rest, elevation, specialized footwear or protective boots, and tailored physical therapy programs. For cheilectomies, patients are weightbearing as tolerated right after surgery in a posteroperative sandal for 1-2 weeks, with full recovery by 4 weeks. For MTP fusions, patients will be protected weightbearing for 5 weeks, with full recovery by 10-12 weeks.

Postoperative Rehabilitation

A structured rehabilitation program is critical to maximizing outcomes after Haglund’s resection surgery:

  • Initial Phase (1-2 weeks): Emphasis on rest, elevation, and gentle range-of-motion exercises.
  • Intermediate Phase (2-6 weeks): Progressive weight-bearing exercises and strengthening routines under supervised physical therapy.
  • Advanced Phase (6-12 weeks): Gradual return to higher-impact activities and tailored conditioning programs to restore full function.
